Making handoffs readable

A handoff should tell the next shift what changed, what is still uncertain, and what requires attention. Long narrative notes are rarely scanned well during pressure.

Use a stable structure: current state, open observations, blocked actions, and named owners. Keep routine completed tasks separate from exceptions.

The goal is shared situational awareness, not a perfect diary.
